The information on this questionnaire is needed in order to pre-qualify and reserve a place on the wait list for an affordable unit.

DO NOT SEND APPLICATIONS BEFORE OR AFTER as they will not be accepted. Successfully completing this form will reserve your place on a waitlist, Completed Pre-Applications will be processed on a first come, first serve basis.

When a pre–application is selected from the waitlist and appears to income qualify for an affordable unit based on the stated information, applicants will then be required to fill out a Full Application, undergo a credit, rental history and limited criminal background check and provide the below items along with their Full Application. Submitting a pre-application does not guarantee that applicant(s) will qualify for a unit at the property. Applicants must be screened and meet the property’s rental criteria, complete an Initial Tenant Income Certification and provide the below supporting documentation before applicant(s) can be approved for an affordable unit.
a. Last 2 years Tax Returns or W-2 Forms, copies of (2) most recent payroll stubs
b. List of all Assets (i.e., real property other than the family residence, checking and savings accounts, stocks, bonds and other forms of capital investment, and retirement accounts or pensions) with current values
c. Income Certification including evidence of any employment income, public benefits such as TANF, SSI, SAA or other benefits, Proof of any non-employment income (e.g. Social Security, Explanation of Benefits or Award Letter, unemployment checks, child support) Proof of Section 8 or VASH vouchers or any other federal, state of local public assistance or housing subsidies. If you have a Section 8 or VASH Housing Assistance Voucher, please provide a copy of the voucher with an active date, if applicable or any other legal, verifiable income.
d. Rental History: Current and Prior Residences.

Consumer Report and Disclosure and Authorization - Credit and Background Check: I understand if selected from the waitlist a background check including credit check and a limited criminal history will be run and I will pay a fee of $33 paid directly by applicant through one of the following organizations:

Should your initial application be selected from the waitlist to Move forward, applicant will have 10 business days to complete application and provide requested back up documentation, if applicant fails to provide information within 10 business days Application will terminate and applicant will lose their place on the waitlist.
